Medical Home Network joins Illinois Medicaid coordination push

CHICAGO, Aug. 14, 2013/ Crain's Chicago Business/ -- A Chicago-based health project has been selected to help coordinate the care of 170,000 Illinois Medicaid patients. 

The Illinois Department of Healthcare and Family Services announced Tuesday that the Medical Home Network is now a "care coordination innovations project."

That means it is part of the state's efforts to increase the number of Medicaid patients in managed care. State law requires at least half of Illinois Medicaid recipients be moved into managed care by 2015. Medicaid is the state and federal health program for the poor and disabled.

Medical Home Network launched in 2009 with initial funding from the Comer Science and Education Foundation. It includes 12 hospitals and 110 primary care sites.
