Building Trusted Relationships with Patients During the COVID-19 pandemic

CHICAGO – Medical Home Network’s Transitional Care Nurse, Mattie Kennedy, RN, BSN, has been recognized as one of Crain’s Chicago Business’ 2020 Notable Health Care Heroes. As a Transitional Care Nurse, Mattie helps manage the care of patients as they transition from the hospital to home – and provides support along their journey to recovery.

Prior to the COVID-19 outbreak, Mattie was visiting her patients across Cook County hospitals, interviewing them, and educating and explaining their next steps for recovery, including plans after they leave the hospital.

While the pandemic has prevented Mattie from in-person hospital visits, she has remained committed to building those trusted relationships with patients virtually. In her role she has especially become a lifeline for patients who need someone to talk to or listen to what they’re going through as they are much more isolated than they typically would be.
