Medical Home Network’s Award-Winning ACO Celebrates 5 Years of Collaboration, Innovation and Impact

Medical Home Network ACO has served Chicago-area safety net patients since 2014

CHICAGO – Medical Home Network’s Accountable Care Organization (MHN ACO), which provides high-quality managed health care to approximately 120,000 Medicaid patients, celebrated its five-year anniversary.

Since 2014, MHN ACO has produced healthier patients while lowering total costs of care through comprehensive care management focused on whole person care supported by Medical Home Network’s proprietary technology and dedicated care manager teams.

“We are proud to recognize this important milestone for MHN ACO and the members who drive our ability to improve patient outcomes,” said Cheryl Lulias, Medical Home Network ACO CEO. “We continue to make collaboration and innovation our priority to better serve safety net patients in Chicago and Cook County.”

In recognition of the five-year milestone, MHN ACO honored three individuals who have played an instrumental role in the success of the ACO – Stephanie Comer, Comer Family Foundation; Dr. Larry Goodman, former CEO of the Rush University System for Health and Rush University Medical Center; and Dr. John Jay Shannon, CEO of Cook County Health.

Earlier this year, MHN ACO announced its contract extension with Cook County Health through 2021. The contract shifts additional financial accountability to MHN ACO, ensuring Cook County Health (CCH) is rewarded for its high-quality care and outcomes for its patients.

MHN ACO is comprised of 13 provider members – ten federally qualified health centers and three hospital systems and their physician practices: Alivio Medical Center, Aunt Martha’s Health & Wellness, Chicago Family Health Center, Erie Family Health Centers, Esperanza Health Centers, Friend Health, La Rabida Children’s Hospital, Lawndale Christian Health Center, Near North Health Service Corporation, PCC Community Wellness Center, PrimeCareHealth, Rush University Medical Center and Sinai Health System.
