Influential Healthcare Leader Dr. Jay Bhatt Joins Medical Home Network as Chief Clinical Product Officer and Medical Director

CHICAGO – Medical Home Network (MHN) announced the addition of Dr. Jay Bhatt, a primary care internal medicine/geriatric physician and public health professional, to its leadership team as Chief Clinical Product Officer and Medical Director.

 MHN provides whole-person care for more than 140,000 of Cook County’s most vulnerable residents – and it is that focus on underserved populations that aligns with Dr. Bhatt’s own passion and why he started a career in healthcare in Chicago.

 Dr. Bhatt comes from a family of health professionals. His father was a pharmacist working in clinics and hospitals serving residents on Chicago’s south side and often visited patients who struggled with choices they had to make between housing, safety and food. It was from these experiences Bhatt realized health care must be accessible, affordable, and equitable for all.

 “I have strong roots in Chicago. It’s where I started to see how we can make positive changes. To once again be able to play an active role in reaching Cook County patients is critically important to me as we navigate the complexities of our healthcare system for the next normal,” said Dr. Jay Bhatt.

 Dr. Bhatt previously served as Managing Deputy Commissioner and Chief Innovation Officer for the Chicago Department of Public Health (CDPH). He led the implementation of Healthy Chicago, the city's public health agenda, and innovations in cross-sector partnerships, predictive analytics, epidemiology and informatics. He was also the first chief health officer for the Illinois Health and Hospital Association before joining the American Hospital Association as senior vice president and chief medical officer and executive lead for several of their business entities on quality and health equity. 

 In his role with MHN, Dr. Bhatt will develop and prioritize the organization’s strategic investments to further develop its clinical and technical assets, continuously rethinking care delivery and how to improve patients’ overall experience.

Dr. Bhatt will also serve as Chief Clinical Product Officer and Executive Medical Director for MoreCare, a Medical Home Network affiliate, and new Medicare Advantage plan delivering healthcare to Medicare eligible persons in Cook County. He will work with the clinical and care management team and provide professional leadership as the direct clinical supervisor, while building collaborative working relationships with facilities and hospitals.

Dr. Jay Bhatt Bio

Dr. Jay Bhatt is a primary care physician, public health professional, and commentator working to care for the underserved and redesign care for vulnerable populations.

Most recently Dr. Jay Bhatt served as Senior Vice President and Chief Medical Officer of the American Hospital Association and President of HRET and interim CEO of the Institute for Diversity and Health Equity. Dr. Bhatt and his team led large national quality improvement and innovation initiatives and worked with key national partners to support interprofessional teams and deploy a model for age friendly health systems.

Previously he was the first Chief Health Officer at the Illinois Health & Hospital Association. In this role, he led large improvement projects including the Hospital Engagement Network (HEN), which were aimed at reducing readmissions and hospital acquired conditions. He has launched several improvement collaboratives; managed the Medical Executive Forum; led the Midwest Alliance for Patient Safety; launched a physician leadership academy and designed and advanced a statewide high reliability initiative.

Prior to the Illinois Health & Hospital Association, he was Managing Deputy Commissioner and Chief Strategy and Innovation Officer for the Chicago Department of Public Health (CDPH). There he led the implementation of Healthy Chicago, the city's public health agenda, and innovations in cross-sector partnerships, predictive analytics, epidemiology and informatics. Under his guidance, the department was recognized nationally in their approach to using predictive analytics to improve food inspections, chronic disease, West-Nile and lead inspections. He also led initiatives that bridged the gap between healthcare delivery systems and public health. For example, he led a groundbreaking initiative with Advocate South Asian Cardiovascular Center in developing the SAHEB (South Asian Healthy Eating Benefits Program), Community Health coalition and a Wellness Prevention Fund with health systems. Under his leadership in partnership with Commissioner Dr. Choucair, the Chicago Department of Public Health received an award from the National Forum for Heart Disease and Stroke Prevention and National Association of County and City Health Officials as local health department of the year.
