Cook County Health to Offer New Medicare Advantage Plan, MoreCare

CHICAGO – MoreCare, a new Medicare Advantage health plan, has partnered with Cook County Health (CCH) to bring more coordinated, individualized care to Medicare-eligible persons in Cook County. 

Cook County Health’s mission is to improve access, quality and care delivery for Cook County’s safety net population. With more than 14,000 patients aging into Medicare each year, CCH will be the provider of a continuum of care to patients, like those currently on CountyCare, looking for Medicare coverage.

“CCH wants to provide a seamless transition and competitive option for patients to consider as they become eligible for Medicare,” said Dr. Jay Shannon, CEO of Cook County Health. “CCH and MoreCare health plan continue to put patients and our community at the center, where they belong.”

MoreCare brings a unique, coordinated approach to healthcare, making it easier for Medicare-eligible patients to get the quality, whole person care they deserve. The new Medicare Advantage health plan will offer four products including a traditional Medicare Advantage Prescription Drug Plan and a chronic condition special needs (C-SNP) plan for Medicare-eligible Cook County residents with a positive diagnosis of HIV. This will be the first HIV-specific C-SNP offered in Cook County.

MoreCare is a fully owned subsidiary of Medical Home Network (MHN). Since 2014, MHN has transformed health care delivery and care management for CountyCare patients through an innovative patient-centered, team-based model.
