Job Summary

Reporting to the Sr. Director of Programs and Administration, the Referral and Training Manager will oversee subspecialty referral requests from network providers and care team members for Westside Health Equity Collaborative (WSHEC) enrollees. He/she will be familiar with the referral process to reach subspecialists at all the participating hospital networks and engage patients in seeking care across the healthcare continuum. The Referral and Training Manager will have a particular focus on behavioral health referrals to psychiatry and resource coordination. He/she will also support orientation, training , and ongoing tracking of community health worker hires for program and budgetary purposes.

Essential Functions

  • Works collaboratively with all subspecialty service areas within partnering BH entities and hospital networks to ensure timeliness, accuracy, and completeness when scheduling patient appointments, tests, and procedures
  • Provides accurate and detailed information to include patient demographics and medical information to ensure patient safety and quality patient care
  • Facilitate psychiatric consultations for primary care provider sites. Manage the scheduling and facilitate communications to ensure patient needs are met.
  • Develop and support curriculum and ongoing education for CHWs and other ancillary staff
  • Track trainings in learning management system and keep a record or all CHW hires for training and budgetary purposes
  • Reviews referrals for urgency and process referrals according to established policies and procedures
  • Accurately and consistently communicates appointment, test, procedure information, and prep to the patient and/or family member
  • Ensures patient has a follow-up appointment with the primary care provider to discuss outcomes as appropriate
  • Manages open referral report and tracks all referral orders until completion; follows up with providers on missing reports for their input as per policy and procedure
  • Addresses any barriers to accessing services
  • Consistently and accurately documents the date and time of appointment, tests, scan, or procedure in the designated systems and according to policies and procedures
  • Demonstrates an understanding of HIPAA and release of medical information
  • Exhibits and promotes a customer service standard of excellence in the performance of all duties with patients, co-workers, and outside partners
  • Other duties as assigned

Competencies

· 3-5 years of healthcare experience with knowledge in medical office administration or clinical care.

· Experience making medical referrals preferred

· Enthusiasm for working in underserved communities

· Ability to engage members of the care team and external community partners effectively and appropriately

· Exceptional communication and collaboration skills

· Ability to effectively engage patients and assess needs and concerns

· Excellent in-person and phone customer service skills, managing emotional and challenging patient situations and responding promptly to patient needs

· Ability to adapt to change; approach challenges with an innovative and proactive attitude, and investigate resources

· Strong verbal and written communication skills required

· Bilingual (English/Spanish) desired
