Manager, Clinical Programs and Subcommittees

In collaboration with the Associate Director of Clinical Programs and Subcommittees, Vice President, Clinical Integration and Innovation and key staff within the Clinical Integration team, this position supports the integration and drives the sustainability of programs implemented across the ACO medical homes. This position is responsible for the collaboration and facilitation of clinical subcommittees consisting of interdisciplinary stakeholders to ensure their success in designing goal-orientated initiatives related to the scope of their charters to result in high quality outcomes. This individual will contribute to creating a culture of provider engagement at all levels of each organization through the employment of evidence-based practices.

Duties

  • Working in collaboration and under the direction of the Associate Director of Clinical Programs and Subcommittees, drives the success of assigned programming and Subcommittees.
  • Contributes to the development and implementation of programs to create value and differentiation from the marketplace.
  • Facilitates programs including projects sponsored by the MHN ACO Clinical Committee, its subcommittees and workgroups. This includes research around topics under consideration, preparation of ad-hoc reporting and analysis, agenda preparation for meetings and preparation of reports and supplementary material for Chairperson presentation to the Clinical Committee.
  • Prepare assessments and recommendations for committees and teams
  • Develops relationships with Subcommittee Chairs, Care Management and Quality leaders and staff across ACO members and with other strategic partners.
  • Facilitates work through successful collaborations with other MHN divisions and most significantly with the Project Management, Product Development, and the Mathematics, Data, and Analytics Team (MDAT).
  • Participates and/or leads the development and delivery of trainings related to clinical programs and initiatives
  • Contributes to the success of regulatory and accreditation surveys through participation in relevant planning activities tied to subcommittee work and programs.
  • Creates documents such as progress report formats, process flowcharts, and other performance tools such as dashboards, control charts
  • Coaches medical home teams to identify opportunities for improvement and to implement actions to achieve goals.
  • Serves on MHN wide workgroups and attends internal meetings representing the CI Division.
  • Completion of other duties as assigned

Qualifications and Requirements

  • Bachelor’s Degree in nursing or an allied health profession, or a master’s degree in a healthcare related field (MPH, MHA)
  • Minimum of 4-6 years of healthcare experience focused on staffing and providing support to physician led Committees
  • Experience in safety net/public health organizations such as ACOs, Hospitals, FQHCs and physician groups
  • Excellent communication skills and demonstrated experience working with physicians, other healthcare team members, and senior leaders
  • Demonstrated ability to work collaboratively with a passion to contributing to a changing health care landscape
  • Ability to work independently and on a team with a wide variety of individuals.
  • Excellent organizational skills and data analytical skills.
  • High level of proficiency in data collection methodologies and ability to create organized and meaningful presentations of data
  • Extensive knowledge in Microsoft Excel and PowerPoint
  • Excellent oral, written and interpersonal communication skill and comfort presenting to a wide variety of audiences including senior leaders and physicians, other internal and external customers; confidence in voicing a perspective and in facilitation of discussions that involve multiple points of view
  • Excellent presentation skills, organizational skills, and time management
  • Strong team skills with a focus on communication and motivational experience
  • Knowledge of disease state processes

  • Ability to adapt to and thrive in a fast-paced environment
  • Experience in use of Microsoft Visio and Project
