The Clinical Integration Program Intern provides general program assistance, planning, and implementation support to multiple projects led by the MHN Clinical Management Team, related to Practice Transformation including Care Coordination, Disease Management, Patient Centered Medical Home and Quality.


  • Creates project templates such as survey tools, progress report formats, flowcharts, and other relevant document
  • Assists with training of practice staff members, especially those serving in the non-clinical care management role such as community health workers, medical assistants, and other relevant positions located at MHN participating clinics and hospitals
  • Assists with coordination of the MHN Clinical Committee, Network Group Meeting, Behavioral Health Meeting, Executive CM Meeting. This includes but is not limited to creating and gathering agenda items and supporting documents, taking meeting minutes, and following up on open task items
  • Researches a wide variety of topics for clinical committee members and the Clinical Management Team
  • Organizes and leads or assists with special projects delegated from the Clinical Management Team, summarizing results and creating presentation; examples include creation of an ACO Virtual Discharge Planning Resource Center and Care Coordination Networking Group, Readiness Review
  • Attends webinars and summarizes journal/news articles to keep team up to date on new strategies for improving performance by provider sites
  • Generates sample materials for use by ACO members including patient and staff educational materials, disease specific references, webinars, presentations
  • Supports Clinical Team on other projects as needed
  • Provides general administrative support to the CI team including fielding phone calls and e-mails from provider sites, scheduling meetings, and completing other general office tasks requested


  • Excellent verbal and written communication skills, including public speaking skills.
  • Strong computer skills, particularly with the entire Microsoft Office suite, including Word, Excel, Access, PowerPoint, and Outlook.
  • Excellent client/customer service orientation.
  • Ability to deal effectively with a variety of individuals and professionals, including patients, clinic staff, physicians, and senior leadership.
  • Ability to work successfully in a team environment.
  • Ability to multi-task, prioritize, and work under deadlines.
  • Proactive, self-directed, and able to approach complex tasks independently.
  • Knowledge and experience in the area of Patient Centered Medical Homes.


  • Bachelor’s Degree with a health care related major or minor preferred, but healthcare experience may be substituted.
