CHICAGO, Nov. 1, 2013/ Medical Home News/ -- In early 2013, a seven year-old boy from Chicago’s West side suffered trauma to his jaw during an accident at school. In order to receive proper treatment, he needed to see a dentist, but, since the family didn’t have a dentist or know where to find one, his father took him to a community hospital emergency room. While similar cases occur all too often, redirecting patients to the appropriate care in a timely fashion can be a practical impossibility in today’s healthcare system.

Not so in this case – because of the unique capabilities of Medical Home Network (MHN), a Chicago-based not-for-profit that makes advanced and personalized care coordination across entities a reality, this boy was able to get into a dentist’s chair within 30 minutes.
